Mississippi Division of Medicaid (Jackson, MS)

PHPG assisted the State of Mississippi in the evaluation and re-design of its Medicaid long-term care pre-admission screening process for the frail elderly and persons with physical disabilities. PHPG’s work included developing a draft instrument based on latest national research, conducting a statewide pilot study, developing a scoring algorithm for eligibility determination through analysis of pilot study data and assisting in creation of a web-based application process.

PHPG also assisted in identification of new long-term care service options for individuals with serious mental illnesses and in development of a Money Follows the Person federal grant proposal to explore options for community-based and consumer-directed care.
